Immediate Results after Subtotal Gastrectomy for Peptic Ulcer and Gastric Cancer

Abstract
A immediate clinical study was made on 92 cases of subtotal gastrectomy for peptic ulcer and gastric cancer in Department of Surgery of Chonnam University Hospital.

Among 92 cases, 9 were intractable, 25 stenotic, 12 massive bleeding, 28 perforated ulcer and 18 gastric cancer.

The results are as follows

1) The ratio between male and female was 3.8:1 and the peak of age incidence was 4th decade (34.98 % ).

2) The ratio between male and female in each group was 2:1 in intractable, 3.1:1 in stenotic, massive bleeding 5:1, perforated ulcer 13:3 and gastric cancer 2:1.

3) The duration of symptoms showed over 10 years (23.930/o) in the majority.

4) Abnormal hemogram was noted in 56 cases (60.865Y.) and anemia in stenosis, hemoconcentration in perforation was peaked.

5) In perforated cases, elapsed time of onset of disease was 12-24 hours (48.85%) in the majority.
6) The ratio between stomach and duodenum was 3.3:1.

7) Subtotal gastrectomy was performed 35.790/o of peptic ulcer operation and 34.617. of gastric cancer operation.

8) Immediate complication after subtotal gastrectomy showed in 24 cases (26.087.) and intractable 227o, massive bleeding 8.33%, perforated 35.711/6, stenotic 1201o, and gastric cancer 44.476 in each group.

9) In the ulcer diagnosed as benign before operation, postoperative pathohistologic examination showed malignancy in 8 cases (10.8 % ) and gastric cancers were adenocarcinoma in the majority. (500%).

10) Operative results were excellant (53.3 % ), good (28.240/o), fair (5.43 % ), and poor(5. 43 % ).
11) Mortality was 7.6% and high mortality in perforated ulcer (14.28%) was noted.
